Awake And Paralyzed During Surgery

The Trauma of Awakening
Awareness under anesthesia ranks second only to death as a dreaded complication of surgery. It is reported to affect 40,00-140,000 patients per year in the US but there is reason to believe that many more have awakened during surgery. Because modern anesthesia consists of three agents - a light dose of painkiller, a paralyzing drug, and an amnesic agent that blocks memory of the experience - most patients do not remember awakening and so do not report it to their doctors. The paralyzing drug prevents any struggle or gesture as sign of distress so the surgeon, the anesthesiologist, and nurse cannot see that the patient is awake. Some experimental studies estimate the rate of awareness may be as high as 44%.

Not remembering does not diminish the pain, fear, and utter helplessness of awakening under the knife and being unable to let anyone know. Nor does it diminish the traumatic effect of that experience. Survivors who finally recall the awakening usually describe an experience in which their center of awareness coalesces outside their helpless body and they watch the scene from above. Their frozen state and accompanying depersonalization seems to go on and on without a clear point of resolution. The return to the body may happen in the recovery room or even later in the hospital room and, rarely, only after days, weeks, or years.

Posttraumatic Consequences
The experience of awakening in a panic during surgery and finding oneself unable to move or cry out creates a dramatic crescendo of survival instincts. The drug-induced paralysis thwarts any impulse to escape and deepens the instinctual freeze response. Awareness during surgery carries the exact conditions known to induce post-traumatic symptoms. The horrors of the experience become embedded in the nonverbal mind with a potential to intrude into consciousness whenever triggered. The suffering individual will not relate the emerging panic to unremembered and wordless experiences during a past surgical operation.

Over half of the patients that remember coming light under anesthesia develop the full syndrome of posttraumatic stress disorder (PTSD). Survivors with or without memory often become phobic about surgery and their avoidance of triggers may generalize to a fear of hospitals or doctors or of white coats. They may suffer attacks of frozen panic or depersonalization, sometimes with clouded states of altered consciousness. They may repeatedly hear the voices of operating room staff. Their conditions are often misdiagnosed as panic disorder, major depression, schizophrenia, or epilepsy.

Treatment of Surgical PTSD
Once the true cause is suspected the condition can be readily treated and cured with trauma therapy. Successful treatment does require processing of the entire traumatic experience so the therapy must be able to access that seemingly forgotten memory. The memory is there even though conscious verbal probes fail to reach it. It was not verbally coded when the person was under the dual effect of the anesthetics and the instinctual trauma response. Even though it was stored in fragments of nonverbal perception it can be processed into a narrative form that will be available to conscious thought.

Hypnosis is very effective in providing access to the traumatic memory fragments. Other approaches that use nonverbal communication can also be effective. These are the creative arts therapies such as art therapy, movement therapy, music therapy and psychodrama. In most applications the person constructs a verbal narrative of the trauma and comes to own it as a finished historical event.

Diamonds are still a girl’s best friend, right? We love the shiny gems. They are the most popular rocks sold today. But what exactly are they, anyway? Where do they come from? What else are they used for?

Diamonds are a mineral in one of the two crystalline forms
of the element carbon. They are the hardest natural
substance man knows. Diamonds are sold as gems, and
used
in industrial applications for smoothing, cutting, and
polishing hard materials.

Diamonds are most famous for crystallizing in the common
colorless form. They may also be tranlucent to transparent
white, yellow, green, blue, or brown. Diamonds have a high
refractive index which is why they are so brilliant and
sparkly after cutting. The familiar shape of the diamond is
the octahedron.

The most brilliant diamonds become gemstones for jewelry
and
other uses. For those that don’t make it to gems, there
are other options. There is bort, which is a more poorly
crystallized or undesirable color and in fragmentary
condition, and carbonados which is gray to black opaque.
Bort and carbonados are used as abrasives for the cutting
of
diamonds and the cutting heads of industrial rock drills.

Diamonds are found in alluvial formations and in volcanic
pipes, filled for most of their length with blue ground or
kimberlite, and igneous rock consisting primarily of
serpentine. Diamond yielding earth is mined by both the
open-pit method and by underground mining. After removal
to
the surface, the soil is crushed and concentrated. Passing
the concentrated material in a stream of water over greased
tables does the needed sorting. The diamond is largely
water
repellent and sticks to the grease and the other minerals
retain a film of water, which prevents the sticking to
the grease. Then the diamonds are removed from the
grease,
cleaned, and graded for sale and use.

The earliest sources of gem diamonds were India and
Borneo.
Some famous diamonds are the Great Mogul, Regent, and
Pitt.
Other famous diamonds include the Hope (blue), Dresden
(green) and Tiffany (yellow). In the early 18th century,
deposits similar to those in India were found in Brazil,
mainly of carbonados. In 1867 a stone found in South Africa
was recognized as a diamond. Within a few years began a
wild
search for diamonds. In 1870-1871, dry diggings including
most of the celebrated mines were discovered.

Synthetic diamonds were successfully produced in 1955; a
number of small crystals were produced when pure
graphite mixed with a catalyst was subjected to pressure of
about 1 million lb per sq in. and temperature of the order
of 5,000-F (3,000-C). Synthetic diamonds now are
extensively
used for industry, mainly due to the ease of obtaining and
lower cost for them. Diamonds are still very popular and
symbolize many things. Their popularity does not seem to
be
dwindling any time in the near future.

The Next Revolution in Home Entertainment - IP Based Multi-Room Distribution

The Holy Grail of distributed audio; unlimited zones and unlimited sources, cost effectively. That milestone will be achieved, and soon, through the combination of digital amplification and IP based distribution systems. There are systems available today that use one or both of these technologies, such as Oxmoor’s Zon system and Netlinks’ Netstreams product line. The trend will only continue to grow.

The beauty of this approach is that digital music is data, and, as such, can be distributed easily via an Ethernet network. The traditional concept of zones virtually disappears, as each amplifier on the network gets a unique IP address, enabling an audio stream to be routed to it. When combined with a digital music server, the number of zones is limited only by the ability of the server to serve independent audio streams.

The advent of cost effective digital amplification, produced by companies such as D2 Audio and Flying Mole, allows a small, highly efficient audio amplifier to be located close to the speakers, either in the room or mounted in an electrical box. The logical evolution of this approach is to collocate amplifier with the speaker, creating an active speaker. Additional benefits are realized by incorporating the speaker as part of the circuit. This enables the designer to engineer the speaker and amplifier circuits for optimum performance as a system.

Low powered designs of this nature could take advantage of power over Ethernet (POE), simplifying system wiring by allowing a single CAT-5 cable to be run from the router to the speaker. This would not supply a large amount of power (about 15 watts) due to the constraints of the 24ga conductors used in CAT-5. However, the efficiencies gained by an integrated speaker / amplifier design would mitigate this concern to some degree. This type of system is sufficient for background music applications in many residential environments.

Higher power systems can use the existing speaker wire, in the case of a conventionally wired system, to carry power. It can / does use the existing control cable, since many systems have been wired with CAT-5 to the volume control location, for the data.

The bandwidth of a traditional 10/100 network allow, for all practical purposes, an unlimited number of zones and sources in a residential environment. Partly because of this, Audio Over Ethernet has been used successfully in the commercial audio and sound reinforcement world for a number of years now. Cobranet, from Cirrus Logic is the most popular commercial AOE system used today. Another popular system is available from Digigram. These systems both use the MAC layer of Ethernet and are not actually using IP for transmission.

For both residential and commercial applications another primary advantage of using Ethernet is the ability to use the medium for transporting both audio and control signals down the same wire. This increases flexibility and reduces cost for both installation and material.

As was true in the commercial world, the distribution of audio (and soon, video) and control via Ethernet will continue to proliferate in residential distributed audio applications. The combination of flexibility, cost effectiveness and capability are to attractive to be ignored.

Go to the run command by holding the windows key and press R. In the run command type secpol.msc and click on OK. If you are prompted to enter the administrative password do so and click on OK. In the local security settings navigate to security settings then local policies and security options.

Set the two first account protection items to No Prompt and select disabled for the remaining items. Close up everything and you are all set.

You will need to restart your computer for the changes to take effect. This feature can be really useful for preventing spyware from installing on your system. So it might be recommended to just do this to your admin account.

Run Command in the Windows Vista Start Menu

Way back in the good old days of XP there was a little button in the start menu called the Run command. I miss it soo. So we are going to show you how to get it back in the start menu of Windows Vista.

To get it back right click on the task bar and go to the properties. The taskbar is the place where your clock and Start menu are. In the properties go to the Start menu tab. In this tab click on customize and put a check mark beside the Run Command and click on OK. That’s it. I know its a bit of an easy one but if you don’t know where to look you wont find it. Anything that was in the list with the Run Command can be added the same way run was.

Enable the Avalon Engine in Windows Vista

Windows Vista inherited a new set of special effects that occur on the desktop doing day to day tasks. The engine that runs all of this and other graphical tasks is called the Avalon Engine. This engine is present in the current beta copy of Windows Vista but is not enabled by default. To top it all off only some ATI graphic cards are only supported.

So to disable the hardware checking to see this new look on another graphic card. Open up your registry editor and navigate to HKEY_local_machine/Software/Microsoft. In this key create a key called DWM. In the DWM key create a DWORD called EnableMachineCheck and give it a value of 0. Restart Windows and you are done.
